In the past rhinoplasty was something that was pursued only by the wealthy or the elite. It was not widely available, and was not affordable to the public either. Early cosmetic nose changes were not as refined as what can be achieved today. People wanted radical changes. They wanted smaller noses so everyone knew that they had their nose done.
Today people want aesthetically pleasing noses that fit their face and that are done so professionally that no one can notice the difference. Since the nose is the focal point of the face, it should look like it belongs to that face. The key to aesthetically pleasing nose is shape and proportions. The nose should fit the face with a nice shape and correct proportions. A short nose does not fit a long face, a big nose does not fit a petite face, and a feminine nose does not fit a masculine face. The key to a good nose job is to change the shape and proportions that stay in balance to other key facial areas such as the chin, cheeks, forehead, and jaw angles.
In some cases, changing the shape of the nose alone still does not give one ideal facial balance. As a result, changing other facial features at the same time may be beneficial. Most commonly, bringing out other facial prominences (chin or cheek augmentation) puts the face in better balance and makes the rhinoplasty result look more significant. Only a qualified and experienced plastic surgeon can take such facial balancing.
Old rhinoplasty methods are also different from today. Very often nose surgery results produced long-term complications associated with eventual collapse of weakened bone and cartilages that make up the nose.
Modern nose surgery methods allow creating a more natural look preserving as much bone and cartilage as possible while making the desired changes and helping to prevent long-term structural collapse. This is mostly important in the nose tip which has no solid bone support but a flexible and pliable cartilage framework.
